How We Use Your Feedback

Your feedback is important and valued.

As a patient or support person using our hospital services, your insights help us make meaningful improvements to the care we provide. 

The Information you provided may be used to:

  • Audit and review our programs and services
  • Identify opportunities for quality improvement
  • Inform future planning and development
  • Link feedback data with other internal improvement processes
  • Evaluate the effectiveness of our care activities

Please note:

Your feedback will remain confidential and will not be included in your permanent health record.

If your comments indicate a potential risk of harm to yourself, or others, we may be legally obligated to report these concerns to the appropriate authorities. 

If you prefer not to be contacted you may indicate that below.

We are committed to ensuring your feedback is used responsibly and accurately. 

Based on the nature of your comments:

  • Feedback is recorded for Quality Improvement data.
  • Feedback may be investigated with the relevant departments as part of clinical incident management.
  • All feedback emails are securely uploaded into our incident reporting system. After uploading, the original email is securely destroyed.
